They could be scared of the competition and volume that could come from our countries. Case in point, its over a year now since India became part of stripe, but they still won't let India participate. They know how Indians are. Indians have an extremely high work ethic, if they let India in, you will see many Indians posting as many as 10 articles a day. Maybe they are scared of how that would change the platform's echo system.

Some top writer once suggested that we (writers from the third world countries) just spam and scam on these platforms. This could also be a reason as to why they are so reluctant in intergrating inclusive payment systems like paypal.

I can imagine that it is also a demographic appeal issue for their investors. Much like youtube pays very low for clicks in countries like ours, medium probably knows that if they allow us to earn money like the rest of our 1st world friends, it would skew the demographics much more to the 3rd world writers because quite frankly, we are more and spread across all continents. This could not be a good look in the eyes of the investors when funding is needed. So, they just make it a little harder for us to join the platform. That's why we game the system,knowing that at some point, we might get kicked off.

I just don't see them addressing this any time soon, unless they miraculously find a way to let us in while maintaining the 1st world demographic as the vast majority.
